PAMHC is looking for a full-time Care Coordinator for the Cherokee, Le Mars, or Storm Lake location. Travel required.  Will provide services to Integrated Health Services members, utilizing a model of healthcare that utilizes a patient-centered holistic approach to coordination of care for the Medicaid eligible mental health population. 40 hours per week, Mon-Fri, with occasional evening work.
