Hospice Patient Care Coordinator RN
REPORTS TO: Hospice Nurse Manager
POSITION SUMMARY: The Registered Nurse (RN) patient care coordinator is responsible for the provision and delivery of high quality, safe and cost effective patient care in fulfilling Cherokee Regional Medical Center’s mission. The patient care coordinator is responsible for coordination of patient care between all services, disciplines and care settings. Ensures implementation of the plan of care that is individualized, accurate and appropriate for the needs of the patient. Coordinates staffing schedule to provide volume and patient-specific need based staffing.
ANTICIPATED SCHEDULE: 40 hours weekly, 8 hours shift, some weekends and on-call shifts
Education:
- Preferred: Bachelors in Science of Nursing
License/Registration/Certification:
- Required: Current Registered Nurse Licensure in the State of Iowa
Experience:
- Preferred: 2+ years clinical experience
- Preferred: 1+ years hospice care experience
Knowledge/Skills/Abilities:
- Excellent written and verbal communication skills, required.
- Highly developed problem-solving skills.
- Ability to adapt to flexible work schedules and frequent interruptions.
- Ability to prioritize to meet deadlines on daily work and special projects.
- Must be able to work independently with minimal supervision. Strong project management skills needed.
- Ability to learn and use various community programs.
ESSENTIAL FUNCTIONS:
- Provides oversight of all patient care services and personnel, making patient and personnel assignments and coordination of patient care.
- Coordinates hospice interdisciplinary team (IDT) meetings and documents ongoing revisions of the patient’s plan of care.
- Coordinates care between hospice IDT members
- Assures safe, quality and timely care is delivered and the patient needs are continually assessed and appropriately coordinated
- Coordinates referrals, intakes and obtains face to face documentation to ensure appropriateness of admission according to Medicare guidelines.
- Assists in orientation and training of hospice personnel, including hospice volunteers.
- Supervises the work of others providing feedback for performance evaluations, disciplinary actions, and interviews
- Serves in on-call rotation to support evening, weekend and holiday patient care.
- Abides by all Medicare standards and regulations
- Completes timely certification of Hospice Item Set (HIS) documents and ensures comprehensive assessments are completed within specific time points.
- Ensures quarterly measures for the Hospice Quality Reporting Program (HQRP) are collected. This includes HIS admission and discharge record submission within specified timeframe
- Reviews hospice Notice of Election (NOE) for each hospice patient admission to ensure accuracy of information for submission to the Medicare Administrative Contractor (MAC) within 5 days of admission
- Performs hospice consultations to ensure appropriateness of admission to the program as described by the Medicare Guidelines
- Performs Quality Assessment Performance Improvement (QAPI) activities to ensure and assess total performance operations. Audits charts regularly.
