Anticipated Schedule:  32-40 hour position.  Primarily 8:00 a.m. to 4:30 p. m Monday – Friday with call every 8th day (includes occasional weekends)

POSITION SUMMARY:  Home Health registered nurse (RN) case manager oversees the treatment of home health patients by assessing, planning, and coordinating home health services in collaboration with the physician, interdisciplinary team, and the patient/family/caregivers, if applicable. Provides skilled nursing care, education, and support to maximize patient independence and promote health in the home setting. 

ESSENTIAL FUNCTIONS:

  • Observes, collects, and records objective/subjective data on admission assessment and initiates plan of care. Coordinates care between healthcare providers and the home health interdisciplinary team
  • Ensures the plan of care is individualized, accurate and appropriate for the needs of the patient. Revises the plan of care as needed and re-evaluates the patient’s health care needs regularly. 
  • Provides skilled care to patients requiring part-time professional skilled nursing services including prescribed treatments. Documents skilled nursing visits at point of care and the appropriate clinical notes on the electronic medical record (EMR). 
  • Observes, evaluates, and reports to the provider the patient’s physical and emotional condition and reaction to medications or treatments. Informs providers of social and physical factors in the environment that affect patient care. 
  • Participates and provides required documentation for interdisciplinary team (IDT) meetings and care conferences.
  • Educates the patient and family on diagnoses, disease processes, and treatments ordered by the provider. 
  • Performs diagnostic tests as ordered and obtains laboratory specimens when indicated.  Educates patient/family/caregiver on findings of tests. 
  • Gives preventive immunization or treatments under medical direction.  Shares in procedures designed to prevent and control the spread of communicable diseases and applies measures for prevention and control as required by local health regulations.
  • Aware of fees associated with the home health services, such as nursing, aide, therapy visits and medical supplies and uses cost-containing measures in delivering care. Ensures that charges are applied appropriately
  • Supervises the home health aide by making a supervisory visit to the patient’s residence every 14 days with or without the aide present when skilled nursing services are provided in the home. Supervisory visits will be made in homes of non-skilled patients receiving personal care at least every 60 days with the aide present.
  • Registered Nurse must complete an annual on-site visit to the location where a patient is receiving care in order to observe and assess each aide while he or she is performing care.
  • Provides in-services to staff, assists with orientation of home health personnel, including training of home health aide skills and observing competence of aide skills.
  • Abides by all Medicare standards and regulations
  • Assists with Quality Assessment Performance Improvement (QAPI) program as requested by the manager
  • Completes OASIS documents timely and ensures comprehensive assessments are completed at the specific OASIS time points
  • Reviews  Strategic Healthcare Program (SHP) data analytics and benchmarking to effectively manage performance, stay compliant, and follow best practices for patient care.
  • Observes and assesses the patient’s home environment for safety and educates on changes in the environment and behavior to ensure safety and mitigate risk for injury.
  • Seeks knowledge and best practices in caring for patients and their families and for personal growth
  • Provides safe, quality, and timely care to the patient and assures their needs are continually assessed and appropriately coordinated.
  • Collaborates with the IDT to meet the needs of the patient and family/caregivers
  • Accepts “on-call” duties as assigned.
  • Assists with orientation of hospice employees and volunteers
  • Required to attend 75% of offered in- services as stated in the Personnel Policies and Procedures of CRMC.
Requirements

QUALIFICATIONS:

Education:

  • Required:  Associate Degree in Nursing or Bachelors of Science in Nursing

License/Registration/Certification:

  • Required:  Current Registered Nurse Licensure, State of Iowa.
  • Required:  Basic Life Support
  • Required:  Current and valid Iowa Driver’s License
  • Other certifications related to needs of clients

Experience:

  • Preferred:  Two+ years nursing clinical experience
  • Preferred:  Home health nursing experience

Knowledge/Skills/Abilities:

  • Excellent written and verbal communication skills, required. 
  • Highly developed problem-solving skills. 
  • Ability to adapt to flexible work schedules and frequent interruptions. 
  • Ability to prioritize to meet deadlines on daily work and special projects. 
  • Must be able to work independently with minimal supervision. Strong project management skills needed. 
  • Ability to learn and use various community programs.
