Job Overview
We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. The ideal candidate will play a crucial role in managing the Minimum Data Set (MDS) assessments for residents in nursing homes and hospice care settings. This position requires strong organizational skills, a comprehensive understanding of medical documentation, and the ability to work collaboratively with various healthcare professionals to ensure optimal patient care.
Responsibilities
- Responsible for the completion and submission of MDS Assessments, CAAs and Care Plans.
- Conduct comprehensive assessments using the Minimum Data Set (MDS) for residents in nursing homes and hospice care.
- Collaborate with interdisciplinary teams to gather necessary information for accurate MDS completion.
- Ensure compliance with federal and state regulations regarding MDS submissions.
- Maintain up-to-date knowledge of ICD-10 coding and documentation standards.
- Review and analyze medical documentation to support case management processes.
- Provide training and support to staff on MDS-related procedures and best practices.
- Participate in skilled utilization and State Case Mix management as assigned.
- Participate in resident care plans and outcomes to ensure quality standards are met and monitor documentation to validate that it supports MDS coding.
Skills
- Proficiency in PCC or similar electronic health record systems.
- Strong knowledge of skilled and long term care practices.
- Familiarity with ICD-10 coding and medical documentation requirements.
- Excellent organizational skills with attention to detail.
- Ability to work effectively in a fast-paced environment.
- Strong communication skills for effective collaboration with healthcare teams.
- Experience in case management within nursing home settings is preferred.

Pay: $73,722.00 – $77,350.00 per year
