Seeking a full-time Outreach Worker that can be based out of the Cherokee or Storm Lake location that would cover Cherokee and Buena Vista counties.  Travel required.  Will primarily involve performing the duties of a Community Support and HCBS Habilitation worker on an outpatient basis with adults with a serious and persistent mental illness. Will provide supportive services, skill building, behavioral intervention, and crisis planning services to individuals in their homes and community settings. Occasional evening work.
