Under the general direction of the Administrator, the Team Leader directs and coordinates health and well being care for residents living at the home. The Team Leader is responsible for the coordination of health care needs, evaluation and improvement of program services; management of staff; and the delivery of high quality services as determined by the residents.

The Team Leader is a caring, organized individual that has excellent communication skills and is a advocate for both the residents and other staff members.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Assure the quality of care for each resident.

2. Ensure ongoing compliance with appropriate local, state, and federal regulatory

3. Direct and supervise scheduling of employees

4. Provide management and communicate with staff

5. Utilize professional communication skills in dealing with staff, public, and residents.

6. Actively promote staff participation, teambuilding, and involvement in the day-to-day operations and decision making.

7. Perform and direct resident care including administration of medications, treatment and other identified areas

8. Ensure a safe and sanitary environment is provided for staff and residents at site.

9. Train employees, conduct counseling interviews and initiate disciplinary action as necessary.

10. Prepare written employee performance evaluations and initiate recommendations for salary increases, promotions, and terminations

11. Monitor and maintain records reflecting resident’s physical and mental condition. Report significant changes in resident’s condition to his/her primary care provider

12. Complete and analyze all incidents/accidents.

13. Ensure that the plans and Level of Care changes are documented

14. Coordinate and complete the admit procedure to the facility

15. Coordinate, complete, and monitor discharge of tenants

16. Willing to be on-call 24 hours a day

This is a full time position, employee must be able to work days, nights and weekends as needed.

Education/Certification/License

Medication Manager Certificate or willing to obtain within 30 days of hire

  • Valid Iowa Chauffer Driver’s License, clean driving record
  • Must be at least 18 years old
  • High School Diploma or equivalent
  • Ability to successfully pass a Pre employment Physical, Drug Screen and TB test
